New Patient Registration

If you live within the Practice boundary and have your medical card, simply bring it to the Surgery and hand it to the Receptionist. 

If you have mislaid your card, we can provide you with the necessary form or you can click on the Pre-registration tab and print off a new patient registration form.

New patients are requested to make an appointment with the Health Care Assistant for a routine health check.

You will be asked to complete a medical questionnaire and to bring a specimen of urine for this initial consultation.
